Instructions for Filling Out the Declaration Form for Reissuing Birth Certificates

In the Appendix issued with Circular 04/2024/TT-BTP, there are instructions on how to fill out the declaration form for reissuing birth certificates as follows:

Section (1) Clearly specify the name trực tiếp bóng đá hôm nay the agency reissuing the birth certificate.

Section (2) Only fill out in cases where the individual requesting the civil status registration does not have/do not provide a personal identification number/citizen identification card/identity card; do not provide complete information on the day, month, year trực tiếp bóng đá hôm nay birth.

In cases where information on “Place trực tiếp bóng đá hôm nay Residence” must be provided, fill in the permanent residence; if there is no permanent residence, fill in the temporary residence; if there is neither, then fill in the current place trực tiếp bóng đá hôm nay residence.

Section (3) Fill in the personal identification number/citizen identification card/identity card (e.g., Citizen identification number 025188001010 issued by the Department trực tiếp bóng đá hôm nay Administrative Management on Social Order on November 20, 2021). In cases where there is no personal identification number/citizen identification card/identity card, fill in a valid replacement document (passport, identity card,...).

Section (4) If born at a medical facility, clearly specify the name and address trực tiếp bóng đá hôm nay the medical facility; if born outside a medical facility, fill in the locality trực tiếp bóng đá hôm nay three administrative levels (commune, district, province) where the birth occurred; if the birth place is not fully known, only fill in the provincial-level administrative locality.

Section (6) Specify the name trực tiếp bóng đá hôm nay the agency that initially registered the birth.

Section (7) Only fill out if specifically known.

Section (8) Mark X in the box if requesting a copy and specify the quantity.

In What Cases Can a Birth Certificate Be Reissued?

Article 24 trực tiếp bóng đá hôm nay Decree 123/2015/ND-CP regulates the conditions for reissuing birth, marriage, and death certificates as follows:

Conditions for Reissuing Birth, Marriage, and Death Certificates

1. Birth, marriage, and death registrations conducted at a competent Vietnamese agency before January 1, 2016, where both the civil status register and the original civil status documents were lost, may be reissued.

2. Individuals requesting reissuing trực tiếp bóng đá hôm nay birth, marriage, and death certificates must submit complete copies trực tiếp bóng đá hôm nay documents and materials related to the reissue.

3. Reissuing trực tiếp bóng đá hôm nay birth and marriage certificates shall only be conducted if the requesting individual is still alive at the time trực tiếp bóng đá hôm nay application submission.

Thus, a birth certificate may be reissued if the birth registration was conducted at a competent Vietnamese agency before January 1, 2016, and both the civil status register and the original civil status documents were lost.

What Does the Reissue Application for a Birth Certificate Include?

Article 26 trực tiếp bóng đá hôm nay Decree 123/2015/ND-CP stipulates that an application for reissuing a birth certificate includes the following documents:

- A declaration form as prescribed, including an affidavit by the applicant stating that the birth certificate was registered but the original Birth Certificate is not kept.- Copies trực tiếp bóng đá hôm nay all documents and materials trực tiếp bóng đá hôm nay the applicant, or other documents and materials containing information related to the birth registration.

Note: In cases where the individual requesting reissuing is officials and public employees, or individuals working in the armed forces, the application includes:

- A declaration form as prescribed, including an affidavit by the applicant stating that the birth certificate was registered but the original Birth Certificate is not kept.- Copies trực tiếp bóng đá hôm nay all documents and materials trực tiếp bóng đá hôm nay the applicant, or other documents and materials containing information related to the birth registration.- A confirmation document from the Head trực tiếp bóng đá hôm nay the agency or unit verifying that the birth registration details such as name, middle name, gender, date trực tiếp bóng đá hôm nay birth, ethnicity, nationality, place trực tiếp bóng đá hôm nay origin, and parental relationships align with the records managed by the agency or unit.
