5. Health facilities issuing discharge notes, birth certificate, confirmation of maternity leave, confirmation of poor postpartum health and confirmation of eligibility to receive social insurance benefits shall:

- It is lost or broken;

- vtv5 trực tiếp bóng đá hôm nay seals appended on vtv5 trực tiếp bóng đá hôm nay confirmations are invalid;
- Information recorded on vtv5 trực tiếp bóng đá hôm nay discharge notes, birth certificate, confirmation of maternity leave, confirmation of poor postpartum health or confirmation of eligibility to receive social insurance benefits is incorrect.
In case of re-issuance, a “re-issuance’ seal shall be appended on vtv5 trực tiếp bóng đá hôm nay discharge notes, birth certificate, confirmation of maternity leave, confirmation of poor postpartum health and confirmation of eligibility to receive social insurance benefits.
As regulations above, health facilities in Vietnam re-issue discharge notes in following cases:
- It is lost or broken;

Pursuant to Article 28 of vtv5 trực tiếp bóng đá hôm nay Circular 56/2017/TT-BYT stipulating as follows:
Grandfather clause
1. Discharge notes, birth certificates, confirmations of eligibility to receive social insurance benefits, injury confirmations and death certificates issued by vtv5 trực tiếp bóng đá hôm nay health facilities within their powers and professional regulations prescribed by vtv5 trực tiếp bóng đá hôm nay Ministry of Health from July 01, 2016 to before vtv5 trực tiếp bóng đá hôm nay effective date of this Circular but their forms, dates, seals or signatures are incorrect, they shall remain valid for granting social insurance benefits.
2. vtv5 trực tiếp bóng đá hôm nay templates of vtv5 trực tiếp bóng đá hôm nay confirmation of eligibility to receive social insurance benefits, discharge note and birth certificate issued before vtv5 trực tiếp bóng đá hôm nay promulgation date of this Circular are effective until December 31, 2018.
